In case you have initiated your application process for a driving licence you can download your Medical Certificate online through the Parivahan Sarathi portal. Here are the steps to do so.

  1. Visit https://sarathi.parivahan.gov.in/sarathiservice/stateSelection.do
  2. Select your state from drop-down
  3. On main menu click “Others”
  4. Select “Print Application Forms”
  5. Enter your “Application Number” & “Date of Birth”
  6. You will be a series of options, select “Print Form1-A”
  7. Download Form 1 A in PDF Form or print as per your convenient

Download Blank Form 1 A from Parivahan Portal:

In case you want to download a blank Medical Certificate to furnish it in offline mode, you can do so from Parivahan Portal. Let us check the steps to download the blank form in PDF format.
