4. Select “All Forms” & you will be redirected to a page with all RTO Forms
5. Click “From 4A” against driving licence category
6. Your form will be downloaded in PDF format

How to Fill Form 4A?

10. If you have been convicted for any driving-related offenses, mention the particulars and dates of such convictions.
11. If you have been disqualified or barred from driving, mention the reason for the same.
12. If you have taken a driving test in the past, mention the details such as date, type of test, etc.

Categories of Vehicles for International Driving Permit in RTO Form 4A:

1. Motorcycles (L1 & L2)
2. Passenger vehicles with maximum weight not exceeding 3,500 kg and not more than 8 seats in addition to the driver’s seat (M1)
3. Vehicles used for goods carriage with maximum weight exceeding 3,500 kg
4. Passenger vehicles with more than 8 seats in addition to the driver’s seat
5. Combination of vehicles not falling in categories (B), (C), or (D) but driven by a licensed driver for those categories.
